ABSTRACT

Objective To explore the application of ultrasound-guided arterial line placement in severe patients with COVID-19. Methods From February to April 2020, we retrospectively collected and analyzed the clinical data of critical patients with COVID-19 with an indwelling peripheral arterial catheter treated by the medical team of Peking Union Medical College Hospital. Patients with ultrasound-guided peripheral arterial catheterization were taken as the study group, while patients whose arterial catheter was placed by traditional palpation were taken as the control group. The puncture condition and complication rate were compared between the two groups. Results A total of 60 severe patients with COVID-19 who met the inclusion and exclusion criteria were enrolled in this study. There were 30 cases in the study group and 30 cases in the control group. In the study group, the success rate of the first catheterization of the peripheral artery (63.3% vs. 26.7%) and the total puncture success rate [(79.43+/- 25.79)% vs. (53.07+/-30.21)%] were higher than those in the control group (all P < 0.05), the puncture times(1.43+/-0.56 vs. 2.50+/-1.28) were less than those of the control group (P < 0.05). The rates of 24-hour disuse (6.7% vs. 30.0%), local hematoma (10.0% vs. 36.7%), occlusion, and tortuous (3.3% vs. 40.0%) in the study group were lower than those in the control group (all P < 0.05). Conclusion Under the three-level protection, ultrasound-guided arterial catheter placement for severe patients with COVID-19 can improve the success rate of catheter placement, reduce puncture times, and reduce the incidence of complications.Copyright © 2021, Peking Union Medical College Hospital. ABSTRACT

Against the backdrop of the rapid development of e-commerce and the severe blow to fresh food e-commerce shopping caused by the COVID-19 Pandemic, this paper discusses consumers' perceptions of fresh food in the e-commerce live streaming mode using data mining in conjunction with a literature review and a questionnaire survey before presenting research findings that analyze influencing factors and product purchase behaviours. The total consumption and frequency of live-streaming consumption(FLSC) are closely examined in consumer behaviours. We investigate product's cost-performance cognitions(PCC), influencers cognitions(IC), logistics cognitions(LC),discounts cognitions(DC), live-streaming platforms cognitions(LPC), on-line interaction cognitions (OIC), live-streaming disadvantage cognitions(LDC), and impulsive consumption cognitions(ICC) as influencing factors. According to the data model fitting results, merchants can refer to research suggestions and optimize their marketing strategies, improve the problems in the fresh food live streaming marketing strategy, and improve consumer satisfaction, thereby promoting the development of fresh food e-commerce. © 2022 IEEE.

ABSTRACT

Background: During the course of an epidemic of a potentially fatal disease, it is difficult to accurately estimate the case fatality rate (CFR) because many calculation methods do not account for the delay between case confirmation and disease outcome. Taking the coronavirus disease-2019 (COVID-19) as an example, this study aimed to develop a new method for CFR calculation while the pandemic was ongoing. Methods: We developed a new method for CFR calculation based on the following formula: number of deaths divided by the number of cases T days before, where T is the average delay between case confirmation and disease outcome. An objective law was found using simulated data that states if the hypothesized T is equal to the true T, the calculated real-time CFR remains constant;whereas if the hypothesized T is greater (or smaller) than the true T, the real-time CFR will gradually decrease (or increase) as the days progress until it approaches the true CFR. Results: Based on the discovered law, it was estimated that the true CFR of COVID-19 at the initial stage of the pandemic in China, excluding Hubei Province, was 0.8%;and in Hubei Province, it was 6.6%. The calculated CFRs predicted the death count with almost complete accuracy. Conclusions: The method could be used for the accurate calculation of the true CFR during a pandemic, instead of waiting until the end of the pandemic, whether the pandemic is under control or not. It could provide those involved in outbreak control a clear view of the timeliness of case confirmations. © 2023 The Author(s)

ABSTRACT

The filtration efficiency deterioration over operating time due to exposure to particles, organic solvents and other factors is an obvious defect of electret filter, especially during the coronavirus pandemic. Electrostatic enhanced method has been proposed as a promising way to improve the filtration efficiency and service time of electret filters without increasing the pressure drop. In this work, the effect of discharge electrode structure and operation mode of the electrostatic enhanced structure(EES) on the filtration efficiency of commercial pleated electret filters were studied firstly, then the EES was installed in a special designed prototype air purifier, two key indicators of air purifiers(clean air delivery rate (CADR) and cumulate clean mass (CCM)) were tested to evaluate the actual performance improvement of electret filters by the electrostatic enhanced method. It was found that the discharge electrode structure had significant influence on the filtration efficiency and multi-wire array electrode was more suitable for the discharge electrode to pleated filter to ground mesh structure used in this paper. The decayed electrostatic charges of pleated electret filter cannot be recharged again through the EES in actual operation condition. The filtration efficiency improvement of the test pleated electret filters was dominantly contributed by particle charging. The filter polarizing induced by the external electric field was helpful in increasing the filtration efficiency when the particles were charged while its effect on uncharged particles was almost negligible. Besides, the actual performance of the prototype air purifier indicated that the EES can alleviate the filtration efficiency deterioration of test electret filters and extend the service life of H11 and H13 filters by more than 3 and 1.5 times respectively according to the CCM test results. © 2023 IOP Publishing Ltd.

ABSTRACT

As a theoretically rigorous and accurate method, FEP-ABFE (Free Energy Perturbation-Absolute Binding Free Energy) calculations showed great potential in drug discovery, but its practical application was difficult due to high computational cost. To rapidly discover antiviral drugs targeting SARS-CoV-2 Mpro and TMPRSS2, we performed FEP-ABFE–based virtual screening for ∼12,000 protein-ligand binding systems on a new generation of Tianhe supercomputer. A task management tool was specifically developed for automating the whole process involving more than 500,000 MD tasks. In further experimental validation, 50 out of 98 tested compounds showed significant inhibitory activity towards Mpro, and one representative inhibitor, dipyridamole, showed remarkable outcomes in subsequent clinical trials. This work not only demonstrates the potential of FEP-ABFE in drug discovery but also provides an excellent starting point for further development of anti-SARS-CoV-2 drugs. Besides, ∼500 TB of data generated in this work will also accelerate the further development of FEP-related methods. © The Author(s) 2022.

ABSTRACT

When public health emergencies occur, a large amount of low-credibility information is widely disseminated by social bots, and public sentiment is easily manipulated by social bots, which may pose a potential threat to the public opinion ecology of social media. Therefore, exploring how social bots affect the mechanism of information diffusion in social networks is a key strategy for network governance. This study combines machine learning methods and causal regression methods to explore how social bots influence information diffusion in social networks with theoretical support. Specifically, combining stakeholder perspective and emotional contagion theory, we proposed several questions and hypotheses to investigate the influence of social bots. Then, the study obtained 144,314 pieces of public opinion data related to COVID-19 in J city from March 1, 2022, to April 18, 2022, on Weibo, and selected 185,782 pieces of data related to the outbreak of COVID-19 in X city from December 9, 2021, to January 10, 2022, as supplement and verification. A comparative analysis of different data sets revealed the following findings. Firstly, through the STM topic model, it is found that some topics posted by social bots are significantly different from those posted by humans, and social bots play an important role in certain topics. Secondly, based on regression analysis, the study found that social bots tend to transmit information with negative sentiments more than positive sentiments. Thirdly, the study verifies the specific distribution of social bots in sentimental transmission through network analysis and finds that social bots are weaker than human users in the ability to spread negative sentiments. Finally, the Granger causality test is used to confirm that the sentiments of humans and bots can predict each other in time series. The results provide practical suggestions for emergency management under sudden public opinion and provide a useful reference for the identification and analysis of social bots, which is conducive to the maintenance of network security and the stability of social order. © 2022

ABSTRACT

INTRODUCTION: While cases of Thyroiditis after COVID mRNA vaccination are reported, the recovery duration is not yet determined. Therefore, we present the time course of two healthy middle-aged South Asian patients who developed symptomatic, biochemical, and radiological evidence of Thyroiditis following COVID 19(mRNA) vaccine administration. Despite being more than six months post-vaccination, both are still hypothyroid, requiring levothyroxine replacement. CASE DESCRIPTION: Case 1: A 35-year-old male presented with palpitations and neck pain ten days after receiving the first dose of the Pfizer-BioNTech vaccine, initial labs showing biochemical hyperthyroidism TSH -0.07uIU/mL (0.450-4.50uIU/mL), Free T 4 (0.82-1.77ng/dL4) -3.04ng/dl, TotalT3 (71-80ng/dl) -200ng/dl. He was started on propranolol and ibuprofen for symptom control. During four months of follow up, he was euthyroid with TSH -2.060 uIU/mL Free T4 1.04ng/dl, Total T3 122ng/dl, and five months of follow up showed hypothyroidism with TSH -5.730uIU/mL, Free T4 -1.00ng/dl, Total T3 109ng/dl. Due to fatigue and weight gain symptoms, he was started on Levothyroxine 25mcg daily. Case 2: A 41-year-old female presented after the second dose of Pfizer BioNTech vaccine four weeks prior. Initial workup confirmed biochemical hyperthyroidism (TSH-0.019uIU/mL, Free T4 -2.52ng/dl, TotalT3-233ng/dl). During five-month follow-up, laboratory workup showed hypothyroidism with TSH -8.570uIU/mL, Free T4 0.98ng/dl, Total T3-111ng/dl, and still requiring Thyroid hormone replacement for the past six months. She was started on levothyroxine 25mcg daily increased to levothyroxine 25mcg nine pills/week to target TSH 0.5 to 5.0uIU/mL. At the time of diagnosis, both had TPO (Thyroid Peroxidase)-negative, TSI (Thyroid Stimulating immunoglobulin)-negative, and no autonomic nodules on thyroid ultrasound. Case 2 also had a Technetium-99m pertechnetate uptake scan, which showed decreased radiotracer uptake in both the lobes of thyroid gland. DISCUSSION: Thyroiditis is an inflammatory disorder diagnosed based on clinical and laboratory findings. While there have been cases reports of Subacute Thyroiditis following immunizations, such as the influenza vaccine, the mechanism of thyroiditis after the COVID mRNA vaccine remains unclear. Subacute Thyroiditis typically manifests as a triphasic clinical course of thyrotoxicosis, followed by hypothyroidism and a return to the euthyroid state;however, mRNA vaccine-induced Thyroiditis seems to have a different disease course. In our observation of these case series, both patients remained in the hypothyroid state requiring Levothyroxine replacement eight-and twelve-months post-vaccination, respectively. Whether they developed permanent thyroid damage or are able to recover with prolonged recovery time is still unclear, which needs further follow-up in the future.

ABSTRACT

Background: To evaluate the efficacy and safety of different treatment modalities of regorafenib in patients with previously treated metastatic colorectal cancer (mCRC) in the real-world setting. Methods: Individual patient data were retrieved from three leading oncology centers in China from January 2016 to March 2021. The primary endpoint was progression-free survival (PFS), and secondary endpoints were overall survival (OS) and safety. Results: The characteristics of patients who received treatment are shown in the table. Twenty-one patients received regorafenib combined with capecitabine as the second-line treatment for those who cannot visit hospital for their chemotherapies because of the COVID-19 pandemic. The median PFS and median OS were 8 (95% CI 4.36 -11.00) months and 26.9 (95%CI 20.54 -NR) months. 101 patients received regorafenib and 69 patients received regorafenib plus immune checkpoint inhibitor (ICIs) as third or higher line treatment, the overall response was 4.1%(7/170), including one complete response. Patients combined with ICIs have longer PFS than those with regorafenib monotherapy (median PFS = 3.3 versus 2.1 months;p = 0.01). Starting dose was 80, 120 and 160 mg in 64, 40 and 39 patients, respectively. Dose reduction was observed in 43.3% (39/79) of patients receiving 120 and 160 mg as the initial dose. Conclusions: Different treatment modalities of regorafenib all showed promising efficacy and safety in the treatment of mCRC. Regorafenib combination is better than regorafenib monotherapy. Regorafenib combined with capecitabine provided a new treatment strategy during the epidemic but requires further investigation.

ABSTRACT

Coronavirus Disease 2019 is an acute respiratory infectious disease caused by Severe Acute Respiratory Syndrome Coronavirus 2, which has posed a major threat to world economic development and people's health and security. In view of the emergence of virus variants, the difficulty of prevention and control is constantly escalating, and rapid, simple and large-scale detection methods play a key role in epidemic control. Based on Fourier transform infrared spectroscopy detection technology, pattern recognition and plasma disinfection technology, this paper developed a new integrated system for the detection and disinfection of pathogens, and preliminarily tested the effectiveness of the system. In terms of 'detection', the data scale was expanded from 115 to 857 cases. Recognition algorithms including partial least squares classification and convolutional neural network were used to establish classification models for the positive, health control and interference samples, and the prediction accuracy could reach 91.97% and 98.29% respectively. In terms of 'disinfection',to reduce the safety risk of the operation safety, a sample drying and disinfection module and a flexible disinfection film were developed based on the plasma disinfection technology, which was used to protect the key positions of the instruments. The disinfection rate of E. coli in both modules could be higher than 99.9%, in line with the relevant provisions. In summary, the two parts of the spectroscopy detection process of Coronavirus Disease 2019 samples have been innovated. For the first time, the combination of 'detection' and 'disinfection' has been realized, which is conducive to the application and promotion of spectroscopy detection methods. © 2022 Chin. ABSTRACT

Social workers in Wuhan, China were among the first to respond to the public health crisis caused by Coronavirus disease (COVID-19) in early 2020. Social workers in Wuhan developed and implemented an effective interventional model integrating online and offline volunteers of multiple professions-the '4 + 1' model-to support affected individuals in the process of battling the pandemic. Transformational social work leadership played a vital role in the widely adopted model in China, characterised by idealised influence-attributed (or charisma);idealised influence-behavioural;inspirational motivation;intellectual stimulation and individual consideration. Contextual performance is also discussed, followed by a discussion on why social work can play a leadership role in inter-disciplinary intervention in the pandemic crisis. The article concludes with the areas social workers can improve on for the betterment of leadership.

ABSTRACT

Due to the COVID-19 pandemic, all schools in China shifted to online learning in the spring of 2020. For many schools located in the rural areas of western China, however, it was their first attempt at online learning, and the critical question regarding the effectiveness of online learning on academic performance remains to be answered. In this study, we tracked 125 Grade 10 students in a rural high school in western China, and we compared their academic performance before and after the online learning practice. In addition, four hierarchical regression models were computed to explore the influencing factors of post-pandemic academic performance in different subjects. The results indicated that online learning did not increase students' academic performance in the rural high school and significant drop of performance was seen in mathematics and English. Students' demographic features had little influence on the change of academic performance, as prior academic performance remained the most important predictor of the post-pandemic academic performance. © 2021 IEEE.

ABSTRACT

A questionnaire was used to investigate the emergency training needs of novel coronavirus pneumonia of disease prevention and control institutions in provinces, deputy provincial level regions and cities specifically designated in the state plan, and the effect evaluation of emergency training activities conducted by Chinese Center for Disease Control and Prevention (China CDC). The results showed that 67.4% of 47 disease prevention and control institutions (31/46) believed that the emergency training at the initial stage of the epidemic should be conducted as soon as possible, and the form of network training should be given priority. The training should focus on the urgently needed technologies such as epidemiological investigation, formulation and response of prevention and control strategies, laboratory testing, etc. The teaching materials should highlight pertinence and practicability and be presented in the form of electronic video. The average satisfaction score of the video training conducted by China CDC was (8.81±1.125) and the score of audio-video courseware was (8.97±0.893). The needs analysis and evaluation of novel coronavirus pneumonia prevention and control in disease prevention and control institutions could provide reference for the follow-up training and improve the emergency training management.

ABSTRACT

SARS, MERS, SARS-CoV-2 and other pathogens have caused many pandemics in the world. These pathogens are often spread as aerosols in the air. Thus, fast, efficient air disinfection is essential for effectively limiting the spread of the pathogens. A low temperature plasma disinfection method that deactivates many kinds of bacteria, fungi, viruses, spores and other microorganisms has attracted much attention due to its efficiency and environmental friendliness. Disinfection methods can be divided into physical disinfection, chemical disinfection and comprehensive disinfection based on their key factors. This paper reviews the disinfection mechanisms, application scenarios, development progress and other characteristics of various disinfection methods. The review then focuses on the application of these technologies to the disinfection of pathogens such as SARS-CoV-2 with emphasis on plasma disinfection including the key methods and prospects of plasma disinfection in central air conditioning systems. Finally, the Gong Zi Ting performance center of Tsinghua University is used as an example to show the practicality of this surface discharge plasma disinfection method as an example for further applications. This method can significantly improve epidemic prevention and control, as well as the construction of national biosafety systems. © 2021, Tsinghua University Press. All right reserved.

ABSTRACT

Effects of Covid19 vaccine (authorized by the Food and Drug Administration and manufactured by Pfizer-BioNTech, Moderna) on thyroid are still not reported. We present two cases of healthy middle aged south Asian patients who developed hyperthyroidism symptoms and confirmed as subacute thyroiditis (SAR) following the administration of COVID 19 (mRNA) vaccines within 2-6 weeks. Interestingly, both of them received PfizerBioNTech vaccine CASE 1 A 35 year-old male with Past Medical History (PMH) of seasonal allergies with palpitations and neck pain 10 days after receiving first dose of Pfizer-BioNTech vaccine. Initial labs after 2 weeks of vaccination showed biochemical hyperthyroidism. Repeat labs trended down. DOSE -1 : 03/26/21, DOSE -2 : 04/18/21 TEST DATE 04/13/21 : TSH (0.450- 4.50uIU/mL) - 0.07uIU/mL , Free T 4 (0.82-1.77ng/dL4) - 3.04ng/dl, TotalT3 (71 - 180ng/dl) - 200ng/dl, TPO(Thyroid Peroxidase) - negative, TSI(Thyroid Stimulating immunog lobulin) - negative, Thyroid Ultrasound - No nodules 04/22/21 : TSH - 0.012uIU/mL, Free T 4 - 2.11ng/dl, TotalT3-198ng/dl CASE 2 A 41 year old female with unremarkable PMH was referred by Primary care physician(PCP) for evaluation of secondary amenorrhea and infertility. She did receive her second dose of PfizerBioNTech vaccine 4 weeks prior. Initial work up confirmed biochemical hyperthyroidism. Repeat tests 4 weeks later trended down with clinial improvement of symptoms. DOSE -1 : 12/2020, DOSE -2 : 01/2021 TEST DATE 02/21 : TSH-0.019uIU/mL, Free T4 - 2.52ng/dl, TotalT3-233ng/dl 03/21 : TSH- 0.006uIU/mL, Free T4 -3.06ng/dl TotalT3- 257ng/dl . TPO -negative, TSI - negative, Thyroid Ultrasou nd - No nodules, Technetium-99m pertechnetate uptake scan - decreased radio tracer uptake in both the lobes of the thyroid gland. 04/21 : TSH - 0.020uIU/mL, Free T 4 - 1.51ng/dl, TotalT3-102ng/dl The mechanism of SAR induced by mRNA covid19 vaccine still remains unclear Some unique characteristics seen in this case study -Incidence - in young and middle age group, both male and female got affected -Ethniciity - both are of South Asian ethnicity -Never had COVID infection in the past -Seen with both the first dose and second dose of vaccination The cross recognition between the coronavirus spike protein targeted with the mRNA vaccine and healthy thyroid cell antigen exists as evidenced by this case. Hence association of COVID vaccination with SAT should be considered as a significant addition to literature.
